Insurance & Financial Information
Our business office can assist you with any questions or concerns that you may have regarding the filing of your insurance claims. We file your primary insurance, as well as your secondary insurance, if applicable. We currently participate with Medicare, in addition to most major insurance plans.
Please feel free to contact the Business Office or your insurance provider to confirm that we are a participating provider for your specific plan. At your initial appointment, please bring your insurance card(s) with you so that we may retain a copy on file. Additionally, please notify the Business Office promptly regarding any insurance changes you may have. Please note that some commercial plans require pre-authorization before services will be covered.
Please remember that you are responsible for any co-payments or deductibles, at the time of service. We accept cash, check, Visa and MasterCard. We will be happy to assist you with an alternative payment plan, if necessary. Please contact the Business Office at 319/739-2046.
